Early Intervention Referral for Pre-school Age Children

Early Intervention Eligibility

Non-school age siblings of students who have been declared homeless are immediately eligible for referral to the Beaver Valley Intermediate Unit (BVIU) Early Intervention program for screening.  Screening determines if the non-school age sibling(s) qualify for early intervention services offered at the BVIU.

REFERRAL PROCESS

  • School Staff, the parent or the homeless liaison fills out the Blackhawk Student Resident Questionnaire (SRQ) when an existing or incoming student has been declared homeless.
  • The person filling out the SRQ form should identify all siblings of the student and their age. If a pre-school age sibling is noted, the homeless liaison or parent will fill out the BVIU Referral for Evaluation for Early Intervention located on the BVIU website at:

  • This referral form will trigger the BVIU screening process to see if the pre-school age sibling is eligible for services.
  • The BVIU will decide and notify the parent or person who submitted the evaluation form if the child is eligible and any subsequent steps.
